Declaration
In terms of the Labour Relations Act No. 66 of 1995, Chapter III read with subsections (1) and (3): I, ________, pay no. ________, hereby tender my resignation from the South African Municipal Workers' Union and cancel any stop order / subscription fees deducted on my behalf, giving one month's notice in terms of the Act as stated above.
